Admissions Form - Skilled and Long Term Care

The support and comfort of our residents' spiritual, nutritional, physical and emotional wellbeing is the primary focus of our long-term care approach. Comprehensive, individualized care plans are designed and evaluated frequently by our Directors of Nursing to insure that we are meeting any changing aspects or healthcare needs in a resident's life.

Daily and weekly religious services are provided on both Hillcrest and Grand Meadows campuses. Personal pastoral visits with residents take place regularly; additionally, our pastor is happy to meet with and counsel residents and families privately.

A full schedule of social activities encourages individual participation. Varied events can include live entertainment by local musicians, flavorful ice cream socials, pet therapy visits and more.

Our two campuses each offer advanced physical therapy centers with state-of-the-art equipment. Certified skilled therapists deliver daily exercise and wellness programs.
